Clean mechanical module-layout refactor. Verified statically: all files moved into client/commands/output module trees with roots declared, no stale pre-refactor paths, credential logic split out of auth with no duplicate definitions, all crate:: imports resolve to new roots, and the main.rs dispatcher is intact. No behavior changes.
